Answer: The safest/best time to purchase it, is a day after which the form is out. Avoid waiting, till the very last minute before purchasing the form. Delta Slate Polytechnic, Otefe – Oghara has scheduled Tuesday 15th, Wednesday 16th and Thursday 17th August, 2015 for the screening of Candidates who wrote the 2015 Unified Tertiary Matriculation Examination (UMTE) for the 2015/2016 academic session.

(1) Candidates Eligible for Screening

(i) All Candidates who chose Delta State Polytechnic, Otefe Oghara as their first or second choice and scored 150 and above in the 2015 UTME,
(ii) Candidates who chose other Polytechnics but wish to change to Delta State Polytechnic Otefe – Oghara and scored a minimum of 150 having changed it through JAMB,

(2) Documents Required for the Screening

Candidates for the screening are required to come with the original copies of the following: 

i JAMB and Internet Result Slip ( you are advised to get the one that bears your photograph, if you don’t have this, click here to get yours.)
ii O/Level Results
iii Certificate of Local Government of Origin
iv. Two recent Passport Photographs
v. The Screening Form (Photocopy)
vi Writing Material

(3) Collection of Screening Forms

Screening forms are available for collection at Admissions Office, Delta State Polytechnic, Otefe-Oghara on the presentation of Bursary receipt of N5,000.00 processing fee,

(4) Screening Exercise

Screening shall take place in all the Departments on the scheduled dates, 15th,16th and 17th September, 2015.

(i) 5 O/L Credits including Mathematics and English Language in subjects relevant to the Candidate’s course of study. CRS is accepted only for all Business/Social Science Courses.,
(ii) The following subjects are taken as one: Economics or Commerce, Government or History, Biology or Agricultural science.
(iii) Candidates must have at least a Pass in Physics for Computer Science,
